Prairie Babies Home Daycare
Welcome to my home daycare. It is my policy to do all I can to provide a safe,clean and happy environment for your child. My home provides a loving,caring and nurturing atmosphere.In my care a child will learn to socialize with other children,and learn to respect others by learning positive ways of communication.My daycare also prepares your child for the first step into pre-school by teaching the child the basic knowledge that he or she needs to have a successful start to their education. Policies All infants in my care will be placed on their backs to sleep during nap time,only under the specific written instructions of both a licensed physician and the child's parents will I act otherwise. Please keep all sick children at home. Children with fevers,undiagnosed rashes, eye infections,vomiting or diarrhea will not be allowed to attend My home daycare until a doctors note stating that the child is no longer contagious or a note stating that the child has started a course of antibiotics is supplied.Or until a 24 hour period has passed with no symptoms. If an illness develops or if your child has been exposed to a communicable disease please inform me so that I am aware of it.If your child becomes ill during the day I will call you immediately and you will be expected to pick up your child no more than one hour later. Biting is not tolerated if a child bites another child or the daycare provider hard enough to leave a mark or draw blood the parent will be called immediately and the parent must come and pick up the child as soon as possible.If a child bites hard enough to leave a mark or draw blood on three occasions then it is the three strikes your out rule, the contract will be immediately null and void and the parent must seek daycare elsewhere and will not be allowed to bring the child back to my home daycare.If a child requires medication for a cold or cough or teething then the parent must write a note giving the daycare provider permission to do so along with instructions for the times and doses required. Please call me immediately if your child will not be attending daycare for the day.I will contact you immediately if I will be closed due to sudden illness,Each parent is responsible for having an alternate childcare arrangement. Holidays I will not be open on the following holidays:
When the holidays fall on a weekday then the usual rate of pay applies. Vacation and Sick days The home daycare provider will have two weeks paid vacation per year. I will give parents two weeks notice prior to my vacation. In the case of the client taking vacation, I request two weeks advance notice and you will be charged the regular daycare fee during your child's absence. Vacation checks must be given to the home daycare provider the last day of the child's attendance before vacation. The home daycare provider will have three paid sick days per year. Once again it is the responsibility of the parent to have a backup child care plan. Termination of service The home daycare provider may terminate the child's enrollment immediately if the child's behavior threatens the physical or emotional well being of one or more of the other children in the daycare home. The home daycare provider may also terminate the child's enrollment immediately if the parent is verbally or physically abusive towards the daycare provider,the daycare children or anyone residing in the daycare providers home. Please also refer to Policies concerning biting. Two weeks notice is required for termination other than stated above by either party. The home daycare provider can terminate service effective immediately if not paid by client within three days of when pay is due. Supplies It is the parents responsibility to supply the following:
The above items can be stored here in the child's cubby or you can bring them daily. Trial period A period of two weeks will be used by the parent and the daycare provider to evaluate the child's comfort and adjustment period. In this two week time frame the parent will also evaluate the home daycare providers performance. In this two week time span the parent and the provider have the option of terminating the contract and childcare services if they are dissatisfied. Reserving a daycare spot for your child A fee of one weeks pay is required to hold a daycare spot for two weeks. A fee of two weeks pay is required to hold a spot for more than two weeks. The above fee is non refundable.It does not go towards the first payment nor is it a payment advance. The above fee is strictly to hold a spot for your child. During this time if you choose to go to another daycare the fee is still non refundable. If any party chooses to terminate care the fee is still non refundable. This fee is a one time fee. Late fee rates As a home daycare provider I like to remind my daycare parents that I also have dinner and cleaning and appointments after work as well as attending my children's school functions in the evening.In order to make sure that I am able to keep important appointments I charge a late fee of 1.00 per minute for any additional overtime which is payable in cash when your child is picked up. I would like to thank you in advance for your understanding in this matter. Agreement I have reviewed the above contract and agree to follow these rules and pay according to the rate schedule. Termination voids this contract. This agreement is effective from__________________to_____________________one year at which time it will be reviewed and updated.
